Don’t Let Documentation Cost You Revenue
Practical guidance for practices managing DMEPOS claims and documentation.
If your practice bills DMEPOS, the chart needs to tell the full story before the claim goes out. Medicare, Medicare Advantage, and commercial payers are looking closely at whether the supply was ordered, medically necessary, and supported by the patient’s condition.
Why This Matters
A claim can be paid today and questioned later. If the payer asks for records, the documentation must show why the patient needed the supply.
When clinical documentation is unclear, claims are more likely to be denied, delayed, audited, or recouped.
What Payers Want to See
Payers need the record to clearly connect the supply to the patient’s condition and treatment plan. The notes should make it easy to understand why the DMEPOS supply was medically necessary for that specific patient.
For example, if a brace is billed, the note should explain the condition being treated, relevant exam findings, why the supply was appropriate, and how it supports the plan of care.
Common Reasons Claims Get Flagged
Claims are often flagged when the documentation does not clearly support the DMEPOS supply being billed. One common issue is a diagnosis that does not appear to match the supply provided, making it difficult for the payer to confirm medical necessity.
